Things to Remember for 2009-2010
- Frisco ISD’s Cafeteria plan year runs from October 1, 2009 through September 30, 2010.
- The Medical premiums will remain the same but there have been some changes in the benefits effective October 1, 2009. See plan summary and new rates under the Benefits and Forms link/Medical for more specific information.
- MGM continues as the Third Party Administrator (TPA) for Frisco ISD for the plan year beginning October 1, 2009 through September 30, 2010. The annual amount that can be set aside for the Medical Reimbursement account continues at $4,000 beginning 10-1-09.
- The District continues to offer the stored value debit card, referred to as the MGM Flex Card for Medical Reimbursement. This card is a VISA so it can be used at doctor’s offices and pharmacies. Claim forms will no longer be required, but receipts should be maintained. There is a $1.50 monthly tax-free fee for the use of this card; a new card company is being used this year so new cards will be issued to all participants. The old cards will no longer work as of October 1, 2009 and they should be destroyed. The cards are good for 3 years; remember that all funds for the current year must be used by 9/30/09 as they do not carry over. See plan highlights for more specific information.
- MetLife continues as the dental carrier for Frisco ISD. The PDP Basic and Enhanced plan benefits remain the same but the premiums will increase slightly effective 10-01-09. See more specific plan information under the Benefit and Forms link / Dental
- The Vision plan with Block Vision will be offered again this year. The benefits and rates remain the same and rates will increase slightly effective 10/1/2009. See additional plan information and rates under the Benefit and Forms link/Vision.
- A new Identity Theft plan is being introduced this year with ID Watchdog. The plan protects against medical, criminal and financial identity theft and will provide resolution in the event you are a victim. More information can be found under the Benefits and Forms link/Identity Theft.
- Unum will continue as the Disability carrier effective 10/01/09. The new Select Plan allows employees to enroll and/or increase their monthly benefit amount up to 66 2/3% of gross salary without evidence of good health. There are some slight changes to the benefits for the coming year. *Guarantee issue amounts are subject to pre-existing condition limitations. - Voluntary Term Life insurance is offered to all employees again this year. Employees may apply up to 7 times salary; (amounts up to $400,000 are guaranteed issue for new employees)- all applicants must complete a health questionnaire to be considered for coverage unless you are a new employee. Employees can also enroll their spouse up to 100% of employee election, and children may be added for coverage up to $10,000. (New employees can enroll without evidence of good health-no exams or health questions are required).
- Loyal American will continue as the cancer carrier effective 10/01/09. This supplemental insurance pays in addition to any other coverage you have. Anyone that wants to add or change the cancer insurance must complete a new application. - The Allstate Heart/Stroke insurance plan continues to be offered to Frisco ISD. The benefits and cost remain the same for the coming year. This insurance also supplements any other insurance you have in the event of a heart or stroke. See the plan summary and rates under the Benefit and Forms link/Heart/Stroke.
- Texas Permanent Individual Life insurance also continues to be available for Frisco ISD employees. This insurance offers level term life coverage to age 100 without any benefit reductions. The insurance can be purchased for spouse, children and grandchildren without having to purchase any on the employee. A plan summary and rates are available by contacting FBS.
