Things to Remember!
1. Frisco ISD’s Cafeteria plan year is currently under a SHORT plan year running from October 1, 2011 through August 31, 2012. The next plan year will start September 1 and run through August 31.
2. The TRS ActiveCare Medical plans will have rate increases and some changes in benefits effective September 1, 2012. See plan summary and new rates under the Benefits and Forms link/Medical for more specific information.
3. A MEDlink supplement is being offered again this year; you can view the presentation on this website to get an understanding of how it works. This plan pays benefits to offset doctor visits and deductibles. See the brochure on the forms link for more specific information. Click here for a plan overview 
4. National Benefit Services will be the new Third Party Administrator, (TPA) for Frisco ISD's Sec. 125 Cafeteria Plan for the year beginning September 1, 2012. The annual amount that can be set aside for the Medical Reimbursement account will reduce to $2500 beginning 9-1-12.
5. The District continues to offer the stored value debit card, referred to as the NBS Flex Card, (Benny Card) for Medical Reimbursement. This card is a VISA so it can be used at doctor’s offices and pharmacies. Receipts should be maintained in the event of an audit. There is a $1.50 monthly fee for the use of this card but this year, FBS will pay the fee for anyone who participates. Each participant will receive two new cards. The cards are good for 5 years; remember that all funds for the current year must be used by 8/31/2012 as they do not carry over; all runout claims, (expenses incurred but not claimed during the plan year) need to be sent to NBS for processing. 6. MetLife continues as the dental carrier for Frisco ISD. There is an additional Safeguard DHMO plan being offered along with the current PDP Basic and Enhanced plans and rates increase slightly effective 9-01-12. See more specific plan information under the Benefit and Forms link / Dental.
7. The Vision plan with Block Vision will be offered again this year. The benefits and premiums remain the same for the coming year. See additional plan information and rates under the Benefit and Forms link/Vision.
8. An Identity Theft plan is being offered again this year with ID Watchdog. The plan protects against medical, criminal and financial identity theft and will provide resolution in the event you are a victim. More information can be found under the Benefits and Forms link/Identity Theft.
9. Unum will continue as the Disability carrier. The Educator Select Plan allows employees to enroll and/or increase their monthly benefit amount up to 66 2/3% of gross salary without evidence of good health. *Guarantee issue amounts are subject to pre-existing condition limitations. See more plan information under the Benefit and Forms link/Disability.
10. Voluntary Term Life insurance is offered to all employees again this year. Employees may apply up to 7 times salary; (amounts up to $400,000 are guaranteed issue for new employees)- all applicants must complete a health questionnaire to be considered for coverage unless you are a new employee. Employees can also enroll their spouse up to 100% of employee election, and children may be added for coverage up to $10,000. (New employees can enroll without evidence of good health-no exams or health questions are required).
11. Loyal American will continue as the cancer carrier effective 9/01/12. This supplemental insurance pays in addition to any other coverage you have. It is offered on a new Group platform this year which means guaranteed acceptance for all enrollees subject to pre-existing condition limitations. The plan summary and rates remain the same. 12. The Allstate Heart/Stroke insurance plan continues to be offered to Frisco ISD. The benefits and cost remain the same for the coming year. This insurance also supplements any other insurance you have in the event of a heart or stroke. See the plan summary and rates under the Benefit and Forms link/Heart/Stroke.
13. Texas Permanent Individual Life insurance also continues to be available for Frisco ISD employees. This insurance offers level term life coverage to age 100 without any benefit reductions. The insurance can be purchased for spouse, children and grandchildren without having to purchase any on the employee. A plan summary and rates are available by contacting FBS.
